Before Your Appointment
All you need to bring before your appointment is a copy of your insurance card and any questions on payment information. Anyone under the age of 18 must be accompanied by a guardian.
First Appointment
You’ll be welcomed by our staff and asked to fill out paperwork with basic insurance and demographic information while waiting in our comfortable lobby area. The intake process takes about 3 to 5 minutes.
Who to Bring
If you are under the age of 18 you will need to bring a parent or guardian to your appointment. Guardians will need to bring documentation that proves they are the legal guardian.
Convenient Locations
Austintown
Our Austintown location is in a medical office building off Mahoning Avenue, just 0.4 miles off OH-11 exit 39.
Boardman
Our Boardman office is located right off route 224 in Boardman, between West Boulevard and Mill Creek Park/Golf Course entrance.
Canfield
Our Canfield office is also located on route 224. It sits right off the road between Tippecanoe and Raccoon Roads.
